Once your customers have arrived at your waivers landing page, they can choose a document to complete.

After clicking the document button the form will open up to be filled out, with similar fields to the one shown below.

Once the fields have been inputted, they have the option to choose if they want a copy of the waiver they have completed to be sent to the email address they supplied on the form. This field is automatically added for legal reasons to all WaiverMaster forms and cannot be omitted. Once they have clicked or left blank the email opt-in, they simply click the large green “Submit” button to complete the process
